* Fees: To make an appointment you must pay a $25 deposit. This will be applied to your office visit fee. If you are a "no show' this deposit will not be refunded. To prevent losing this deposit please call us one business day in advance if you need to reschedule your appointment.
* Fees: Our fees are separate from the fees you pay the State of Florida to renew your medical license.
